The below items are required to be completed in full prior to participation in try out.
1 VHSL Physical Form 2024-2025 School Year
All physical examinations must take place after May 1, 2022 for a student-athlete to participate in sports at Forest Park High School for the 2020-2021 school year. The examination results, all information, physician's signature, stamp from physician's office, parent's signature, and athlete's signature must be on this form and submitted to the activities office prior to tryouts as well as out of season conditioning. 2 On-Line Concussion Training PWCS
All student athletes and a parent or guardian are required to complete annual concussion education as required by Virginia law. More information can be found in Prince William County Public Schools (PWCS) Regulation 759-1. Student athletes must complete the concussion education with a parent or guardian.
The annual concussion education requirement can be met by taking the online concussion education starting July 15. In-person concussion education is also held several times each year. Families with students participating in multiple sports only need to complete the training for each student once each school year.
3 Emergency Permission Form (BLUE CARD)
Once completed, parent/guardian must sign the bottom of the form on both sides, then turn in hard copy to the activities office. NO electronic submissions will be accepted. New card must be submitted for each sport. You can obtain these from the Activities Office or Main Office.

* Virginia Asthma Action Plan
Per PWCS Reg 757-5 A Virginia Asthma Action Plan (VAAP) must be completed each school year. The VAAP permits a student with a diagnosis of asthma to possess and self-administer inhaled asthma medications during the school day, at school-sponsored activities, while on a school bus, during any Prince William County Public Schools extended day or overnight field trip, at school age child care (SACC), or other school property provided the following conditions have been satisfied:
* STUDENT ACCIDENT INSURANCE PWCS
Many parents without insurance coverage worry about protecting their children in the event of an illness or injury. Optional student accident insurance is an inexpensive way to provide medical coverage in such events. Student accident insurance is not just for those without health insurance. Optional student accident insurance is a secondary level of coverage for those students who are already covered under a parent or guardian’s primary insurance plan. In such cases, the optional student accident insurance may cover expenses not paid for by the primary insurance plan such as co-pays and other out-of-pocket expenses.
